Care Service Application

*Application procedure:

  • Public funds: Apply to the township (town, city, or district) office where the applicant registered his/her address, or a long-term care management center; the municipal or county (city) government will submit the application to the Nursing Home after reviewing the application.
  • Self-paying: An applicant himself/herself, his/her relatives, or representatives shall come to the Nursing Home to make an application directly.

*Application methods:

  • Online Application: Make an application on our website.
  • Application by mail: Download the care service application from our website, and mail or fax the completed form to our Department of Social Workers Section.
  • Apply at the counter: Please visit our Department of Social Workers in person to make an application.

*The following documents must be enclosed:

  • The completed care service application form.
  • Proof of lower middle income or low income (applicable to those who have recourse to public funds. Also, please apply in advance in accordance with the aforementioned procedure for making an application for public funds)
  • A copy of national identity card.
  • Household register (with entire members) (applicable to those who have recourse to public funds. Also, please apply in advance in accordance with the aforementioned procedure for making an application for public funds).
  • Moderate dementia or above with a certificate of diagnosis or proof of physical and mental impairment (when making an application for dementia care services).
  • A physical examination form issued by a public or regional hospital in the past 3 months. The physical examination shall be carried out in two stages. The first-stage medical examination shall include at least a chest X-ray examination. If an applicant contracts a statutory infectious disease, he/she shall submit a certificate of diagnosis stating that he/she is no longer contagious. In the second stage, an evaluation on his/her eligibility will be conducted. The results of his/her fecal examination should be provided 1 week prior to arrival at the Nursing Home (the examination should include bacillary dysentery, amoebic dysentery, and parasitic infections).

      1. A senior who uses diapers needs to pay an additional NT$2,000 per month. Day care costs are NT$40 per day (if a senior stays overnight for a reason, his/her family members should prepare his/her diapers).
      2. In the case of staying less than a month (such as check-in and check-out), diaper costs for the current month will be calculated as follows: NT$2,000 divided by the number of days in the month and multiplies the number of days staying in the Nursing Home
      3. Free of charge for self-pay residents who signed the old version of the contract before May 10, 2009.

      Meal costs A senior who takes leave for more than 2 days (calculated from the day on which the leave starts to a day before it ends) in accordance with the regulations for medical treatment or other legitimate reasons should enclose a request for leave when applying for an interest-free refund of the daily meal costs during the leave period (only applicable to those who signed a care or long-term care agreement after January 19, 2010).

      Care costs A senior who has been hospitalized for more than 7 days a month (calculated from the day on which the hospitalization starts to a day before it ends) for medical treatment should enclose a request for leave or a certificate of hospitalization when applying for an interest-free refund of the daily care costs during the period of hospitalization.
      Diaper costs A senior who has completed the application procedure, but has stayed outside the Nursing Home for more than 2 days, should enclose a request for leave when applying for an interest-free refund of daily diaper costs during the leave period.

      Meal costs A senior who takes leave for more than 2 days (calculated from the day on which the leave starts to a day before it ends) for medical treatment or other legitimate reasons can apply for an interest-free refund of daily meal costs during the leave period (calculated based on the number of days of the month if costs for a full month have been paid, and on the number of days if only partial costs have been paid). However, the amount to be refunded shall not exceed the amount paid in the current month.

      Care costs A senior who has been hospitalized for more than 7 days (calculated from the day on which the leave starts to a day before it ends) for medical treatment or other legitimate reasons can apply for an interest-free refund of daily care costs during the period of hospitalization (calculated based on the number of days of the month if costs for a full month have been paid, and on the number of days if only partial costs have been paid). However, the amount to be refunded shall not exceed the amount paid in the current month.
      Diaper costs A senior who has completed the application procedure, but has stayed outside the Nursing Home for more than 2 days, may apply for an interest-free refund of daily diaper costs in accordance with the number of days staying outside the Nursing Home.

      Care costs A senior who doesn’t come to the Nursing Home due to illness or other legitimate reasons should enclose a request for leave or a certificate of hospitalization when applying for an interest-free refund of daily care costs (based on the number of days).
  • Supplementary notes
    1. Care costs include meal, accommodation, and care costs. Consumables such as daily necessities, nutritional products, diapers, and medical consumables, as well as expenses incurred due to medical treatment or other personal reasons shall be borne by a senior’s family members or Party B in the agreement.
    2. An ambulance of the Nursing Home is provided for use by the elderly for emergency medical treatment (in hospitals in Changhua downtown and Changhua Hospital under the Health and Welfare Department only). If a senior has to go to a hospital for a general outpatient visit, a return visit or a visit to a special department (such as dermatology, otolaryngology, etc.), he/she must go to the hospital by himself/herself. His/her family members must apply for a leave of absence on his/her behalf and he/she should go to the hospital by himself/herself. If he/she makes an application for use of an ambulance for a special reason, he/she must pay for such use in accordance with fee standards formulated by the Nursing Home.
    3. If a senior is to be admitted to the Nursing Home, his/her family members must sign an agreement with the Nursing Home on his/her behalf, have the agreement notarized, and pay a deposit (to be refunded without interest during check-out).
    4. Care costs for the month in which a senior is admitted to the Nursing Home will be calculated based on the number of days they actually stay. Daily care costs are calculated as follows: care costs for a full month divided by the number of days in the month. Monthly care costs shall be transferred to our Bank of Taiwan account or paid directly to us by the 10th of each month.
    5. Since the number of staff will not be reduced and hardware facilities will not be closed when a senior takes a leave of absence or is hospitalized, while another senior cannot use his/her bed, accommodation costs are non-refundable.
