What are the qualifications for applying to the center?

A

applicants' qualifications are:

(1)

ROC citizens over 65 years old, if fits one of the following qualifications , will be accepted by the center :

  1.with chronic disease, and need medical attention
 

2.not being able to take care of themselves and have to depend on others to take care of them, or need Nasogastric Tube, Foley catheter nursing service Those over 60 but not yet 65 and fit one of the above qualifications, if choose to pay for themselves, the institute may decide to take them in or not, depending on their internal facility situation.

(2)

Those with any of the following qualifications will be denied by the center:

 

1.with statutory communicable diseases and may cause group infections if admitted to the center.

 

2.with mental illness that is under category 1 to 4 according to caring-system responsibility form for mentally-ill patients.

Can people with dementia be admitted to the center?

A

The center is specializing in taking in people with chronic disease or are paralyzed that they have to depend on others to take care of them.
If the senior have dementia, but with sound movability, we suggest that they apply to institutions specializing in taking in only senior with dementia, ensuring that they receive professional and appropriate care service.
Only when senior with dementia lose their mobility and have to depend on others to take care of them may apply to the center, depending on their conditions.

When senior citizens apply to the center, what is the application procedure and required documents?

A

The center's caring services are divided into: service paid by government expenses and service paid by patients, and the application procedure and required documents are:

(1)

Senior citizens whose expenses are paid by the government – their applications and required documents will be submitted to the center by the township, city or district office where they registered by following the administrative system and referring to the municipal or county (city) government:

3. I.D. Card

 

4.Certificate of Registered Low-Income Family.

 

5.Health-check form issued by public or regional hospitals within the recent three months and the examination should at least include x-ray check, stool test for Shigellosis, Amebiasis, and parasites infection.
Those with statutory communicable disease should attach a diagnosis certificate, stating that the applicant is no longer infectious.

(2)

Senior citizens pay for themselves or senior citizens that need long-term care:
the applicant or his / her family should submit the following documents to the social work department of the center:

1.Application form

2.Health-check form issued by public or regional hospitals within the recent three months and the examination should at least include check x-ray, stool test for Shigellosis, Amebiasis, and parasites infection.
Those with statutory communicable disease should attach a diagnosis certificate , stating that the applicant is no longer infectious.

How much does the center charge for its care-service?

A

(1)

Seniors whose expenses are paid by government expenses or need long-term care:

 

Seniors from registered low-income families in various county/ city government, their expenses at the center will be covered by the government.

(2)

Seniors pay for themselves or need long-term care:

 

The expense for the care service is NT18,600/per person , per month.
Those need paper diapers, an additional NT2,000 will be charged.
When being admitted to the center, the applicant need to submit an additional NT37,200 (two months’ expenses) as the deposit in the first month, amounting to NT55,800.
The additional NT37,200 will be reimbursed without any interests to the family of the applicant when he / she is discharged.  

(3)

Payment per day:NT300 / per person / per day
(transportation of the patient is be provided by his/ her family).

(4)

Temporary stay:NT620/per person / per day
(transportation of the patient is to be provided by his/ her family).

Q

Any discounts for applicants of meddle-low income families?

A

People of meddle-low income families, if qualified for severe cases of dementia and moderate cases of dementia (must provide the evaluation certificate from family support, and his / her application must be reviewed by the referring county / city government) are considered patients whose expenses are paid by the government; for slight cases of dementia, expenses are the same as self-paid patients.

What are the payment methods?

A

The center, in order to save family members from coming back and forth, provides several payment methods for family members to choose the one that best fits them. Here are the payment methods:

Q

If a senior citizen refuses to stay in the center, but there is no family members available to take are of him / her, how does the center deal with this problem?

A

The purposes of the care center are to solve the problem of senior caring in the family. However, considering the willingness of seniors, if there is not enough people to look after him / her, but the senior himself/herself is not very willing or even rejecting to stay in the center, then, the center will send someone to do positive communication with the senior so that he / she will understand that the professional services the center provides in daily life and technical services are actually beneficial to their quality of life.

Will the center take in the senior less than 65 years old?

A

According to the directions of taking-in affairs, the center is for taking in the senior over the age of 65; however, according to Article 10 of Enforcement Rules for Senior Citizens Welfare Act, for senior citizens over 60 but not yet 65 years and are willing to pay for the expenses themselves, welfare institutions should provide long-term care, accommodations or other services to them, depending on their internal faculty situations. Thus, under the condition that there are still beds available within the institution and his / her family is willing to pay for the expenses, senior citizens over 60 but not yet 65 could be admitted to the institution after being approved by it.

Can senior citizens take days-off from the center if he / she would like to go home?

A

Yes, they can. It is common for senior citizens to be homesick.
So, in addition to encourage their family members to come to visit them, the center also allows family members to take senior citizens home for a few days. The center has its own rules for taking days-off.
When applying for it, please fill in an application form and following the procedure accordingly. Here are the refund policy for the care fee:

Leaving the center because of illness or other just causes, if more than 2 days are taken off by following the proper procedure, food expenses without interests will be refunded to the family on a prorated rate.

(it is considered the rent and may be deducted when applying for personal income declaration.)

If days staying at the hospital because of illness are more than 7 days (including 7 days), expenses will be refunded without interest on a prorated basis.

Does the center only take in senior citizens from Chang-Hua area ?

A

No.
The center will take in any senior citizens who are ROC citizens registered in ROC as well as his / her qualifications are approved by the center. However, due to geographical reasons and the convenience for family visitation, it is suggested that senior citizens to be placed at the caring centers close to their home.

What about the funeral arrangements for senior citizens whose expenses are paid by the government?

A

Since senior citizens whose expenses are paid by the government are mostly alone and have no families, and are referred to the center by county / city government, their funeral arrangements are made according to the regulations of directions for social welfare organizations under the Ministry of Interior on funeral arrangements for senior citizens supported by government expenses. When senior citizens arrive in the center, a social worker assigned by the center will help him / her make a dictated will, stating his / her funeral arrangements after his / her death. Currently, funeral arrangements made by the center is cremation first, then, bone ashes will be placed in the memorial pagoda. Later the deceased's spirit tablet will be placed in the ancestral temple and offering ceremonies will be held regularly.

A

Regarding senior citizens that need regular kidney dialysis, if they are qualified for the center, they can still apply. However, the center is not equipped for kidney dialysis, so his / her family needs to make arrangements for kidney dialysis themselves.

Does the center provide rehabilitation service?

A

The center has a rehabilitation room and has several rehab counselors as the staff. When senior citizens first arrive in the center, those who need rehabilitation service will be issued a rehabilitation consultation sheet by our medical staff. The rehab counselor will then proceed with rehabilitation once the patient is examined by doctor.

How is the medical quality of the center?

A

The center is working with nearby hospitals such as Changhua Christian Hospital, Show Chwan Memorial Hospital, Chang Bing Show Chwan Memorial Hospital, Han Ming Hospital and Hongren Hospital, thus, has very good medical teams providing quality medical service to senior citizens in the center.

Are there any regulations for family visitation?

A

Visiting hours are from 9:00am to 9:00pm. The center welcomes family to pay visits to senior citizens, and accompany them so that they may have some quality time their families. The center also has some regulations of visitation and families’ cooperation is appreciated.

Does the center provide any service to help senior citizens have necessary nutrition?

A

The center has two dieticians as the staff, responsible for preparing nutritious diets to all senior citizens in the center. After a senior citizen arrives in the center, one of the dieticians would pay him / her a visit within three days in order to gain a full understanding of his/her food preferences and special nutrient needs, thus, provides a diet that best suits him / her. At the meantime, health-diet counseling and consultation on special diets for senior citizens in the center is ongoing to ensure that all senior citizens have a nutrient-balanced diet.

What diets does the center prepare for its senior citizens?

A

Diets that the center provides include: full diet, soft diet, semi-liquid diet, diabetic diet, tube feeding, special tube feeding, and vegetarian diet.
There are three courses and one soup for each meal. Breakfast also serves soy bean milk (or rice & peanut milk), and steamed buns. Fruits are served at lunch; night snacks are provided a night. The center provides at least one happy meal every week. In addition, happy snacks and pig knuckle thin noodles are served in birthday parties. The center provides a menu that is both multiple and seasonal, all prepared by our dieticians to ensure deliciousness and health.

A

The center, in order to save family members from coming back and forth, provides several payment methods for family members to choose the one that best fits them. Here are the payment methods:

Maintain ideal weight.

(2)

To reduce fat and cholesterol intake in the diet.

To reduce salt intake in the diet.

(4)

Eat more fiber-rich food.

(5)

Drink more water.

(6)

Eat several small meals every day.

(7)

No excessive tea, coffee or alcohol.

(8)

Balanced intake of all 6 food groups.

Does the center provide emergency medical service?

A

The center is equipped with 2 ambulances, and 4 drivers. In addition to that, the center is only 10 minutes away from its specially-appointed hospitals by car. When emergency incidents happen to patients (supported by government expenses), the center is able to take him / her to the hospital within the shortest period of time. As for patients who pay for themselves, the center will first contact their family and ask them to take the patient to the doctor. However, if the family won’t be able to get to the patient in time, the center will assist in taking the patient to the hospital downtown first.
